Navigating health and citizenship

Facing significant health challenges and language barriers, Raul was struggling to manage his chronic conditions. Already living with poorly controlled diabetes, hypertension and the effects of multiple strokes, Raul’s health took a turn for the worse when recurrent kidney infections led to hospitalizations. His limited vision, family stress and general distrust in systems made it difficult for him to navigate his care.

Upon becoming a WellSense member, the WellSense Transitions of Care (TOC) team stepped in to support Raul. Amir, a bilingual TOC nurse, began working with Raul. Gaining his trust wasn’t easy—after several outreach attempts, they began building a relationship. Over time, Raul started engaging with his care plan and taking steps to improve his health.

Through patient education and constant encouragement, Raul started regularly checking his blood glucose and blood pressure. His average blood sugar readings dropped from 500 to 200 and he learned how to take action based on his blood glucose readings. Additionally, Amir identified a gap in his care and advocated for services like a rollator walker and physical therapy. With the transportation benefits offered by WellSense, Raul began attending his appointments.

Beyond his physical health, Raul faced another pressing challenge: losing his work permit and citizenship status due to his health setbacks. Amir partnered with Bennie, a WellSense Community Health Worker, to connect Raul with a legal aid service to assist with resources and guidance. Raul felt supported in every aspect of his life.

Today, Raul continues his journey toward better health and stability with his WellSense Care Management team. As he expressed during his last call with Amir, “You are my angel.” Thanks to WellSense, Raul is improving his health and gaining hope for a brighter future.
